NWB in cast (max plantar flexion) x 2 wks; boot with 40° heel lifts: 25% WB wk 2-3, 50% wk 3-4, 75% wk 4-5, FWB wk 5-6; wean boot ~wk 8
Key Restrictions
Must begin within 48 hours of injury; avoid DF past neutral x 8 wks; avoid tendon stretching x 6 months; no sprinting/cutting/jumping x 6-9 months; must be complete mid-substance rupture (not avulsion)
Timeline & Phases
4 phases: Acute (0-2 wks, NWB cast), Intermediate (2-8 wks, boot with heel lifts), Advanced Strengthening (8-12 wks), Progress Activity (12-16+ wks). No-contact RTS 6-9 months, full RTS 12 months
