Ankle Fracture — ORIF

Orthopedic & Sports Medicine Specialists (OSMS)

Weight-Bearing
Functional WB exercises initiated wk 8; CAM walker may transition to removable ankle brace at wk 8
Key Restrictions
Do not overstress healing structures; progression is time- and criterion-based per patient impairments; with syndesmotic fixation adjust timeframes pending hardware removal
Timeline & Phases
Phase I (begin at 6-12 wks post-op): continue fracture healing without overstressing structures, normalize ROM, enhance dynamic stability via neuromuscular control, gradual return to functional/light work activities, initiate functional WB exercises at wk 8, initiate balance/proprioception exercises, advance ROM intensity if indicated, may transition CAM walker to removable ankle brace at wk 8; Phase II (begin at 12+ wks, criteria: minimal/no pain, full AROM without substitution, 5/5 strength, non-antalgic gait): full P/AROM, enhance dynamic stability, gradual restoration of strength/power/endurance, advance neuromuscular control, return to full ADLs/work, may initiate impact and sport-specific movements with physician approval
