Core Decompression — Hip Core Decompression (Arthroscopic-Assisted)

- Weight-Bearing
- TTWB (20 lb) x 4 weeks; progress to WBAT by weeks 4-8
- Key Restrictions
- Limit ER <20 deg x 2 weeks; no hyperextension; no hip flexion isometrics until week 3; no running until cleared; no RTP without MD clearance; CPM 4 hrs/day x 2 weeks (reduce to 3 hrs/day if using stationary bike)
- Timeline & Phases
- 0-4 wks: TTWB, CPM, pain-free PROM, hip isometrics (no flexion until wk 3), core strengthening; 4-8 wks: progress WBAT, DC brace, normal gait, elliptical, hip strengthening isotonics; 8-16 wks: return to ADLs, light plyometrics, running if MD cleared at 12 wks; 16+ wks: advance plyometrics, full RTP with MD clearance
