Phase I (wk 1): posterior splint; Phase II (wks 2-6): ROM brace, shoulder/wrist/gripping exercises; Phase III (wks 6-10): discontinue brace wk 8, shoulder/wrist isotonics; Phase IV (wks 10-16): UBE; Phase V (wks 16-26): light biceps isotonics, plyometrics (two-handed wk 16, one-handed wk 20-22); Phase VI (wks 26+): sport-specific training and return to activity
