WBAT; walking up to 1/2 mile/day (15-30 min) Phase 1; progressive ambulation thereafter
Key Restrictions
No lifting >5 lbs initially; avoid lifting/bending/twisting; Jewett brace if >2 levels or osteoporotic; no dynamic abdominal exercises, twisting movements, trunk flexion with weight, explosive/high-impact loading if osteoporotic; avoid sitting rowing and leg press (anterior column loading); low-load/high-rep emphasis for osteoporosis
Timeline & Phases
Phase 1 (0-2 wks): home HEP, ankle pumps/glute sets/abdominal isometrics/SAQ/diaphragmatic breathing, walking to 1/2 mile/day; Phase 2 (2-4 wks): OP PT, multifidus recruitment, balance, aquatics, gait training; Phase 3 (4-8 wks): advance lifting to 15+ lbs, thoracolumbar stability progression, advanced cardio; discharge criteria: MMT functional, AROM symmetrical, Oswestry >20
