Key Restrictions
No lifting/pushing/pulling; WHFO (wrist neutral to slight extension, MCP in extension allowing IP flexion); no ulnar deviation of digits; no resistance strengthening until 8 weeks; no composite wrist and finger flexion until week 8
Timeline & Phases
Phase 1 (0-2 wks): volar wrist/hand splint; Phase 2 (2-6 wks): WHFO full time, IP AROM, week 4: short arc 0-30° MCP with hook emphasis, AROM wrist; Phase 3 (6+ wks): wean orthosis during day/continue at night until 8 weeks, composite fist and wrist flex from week 8, grip strengthening from week 8; RA may need long-term anti-ulnar deviation support
Notes
For MCP arthroplasty WITH EDC centralization; RA patients may need long-term soft neoprene anti-ulnar deviation orthosis; OA patients wean orthosis as tolerated; additional full digit support attachment for night especially if multiple digits; short arc MCP ROM at week 4 with emphasis on extension in hook position
