Limit IR during early phases (protects posterior capsule — opposite of anterior Bankart); no IR x 6 wks; sling 4-6 wks; no activities above head or across body wks 0-4; avoid posterior-glide joint mobilizations x 8 wks; no horizontal abduction behind body; no push-ups until wk 12; horizontal adduction across body begins at wk 8; RTS non-contact athletes 4-5 months, contact athletes 6 months
Timeline & Phases
5 phases: Maximum Protection (0-4 wks, sling, PROM ER to tolerance, no IR), Protected Motion (4-6 wks, no IR, PROM flexion to 125°), Intermediate (6-12 wks, AROM at 6 wks, IR progression to 45° wk 8, 65° wk 10), Advanced Strengthening (13-24 wks, plyometrics, interval throwing). Total 21-28 wks; RTS non-contact 4-5 months, contact 6 months
