Key Restrictions
Sling x 4-6 weeks (remove 3x/day for exercises); ER limited to 45 degrees until 12 weeks; IR limited to 45 degrees (arm at side and in 90 deg abduction) until 12 weeks; no shoulder pulleys without supervision until 6 weeks
Timeline & Phases
0-6 wks: sling (remove 3x/day), pendulums, supervised PROM (flexion 60 deg, ER 45 deg, abduction 90 deg, IR to neutral), sub-max RC isometrics, scapular strengthening; 6-12 wks: ok to run, advance ER PROM to full, Theraband ER, wall walks, isotonic RC (up to 6-8 lbs), limit IR to 45 deg; 12-24 wks: eccentric RC, isokinetics, weight training, return to sport 3-6 months; Throwers: isokinetic testing at wk 12 (ER/IR ratio >=70%, ER bilateral >=98%, IR bilateral >=105%), then interval throwing program
