Weight-Bearing
TTWB (20 lb through heel with walker/crutches) x 6 weeks; progress to WBAT at 6-8 weeks; limit ambulation <1 hr/day Phase 1
Key Restrictions
Avoid hip adduction across midline; no repetitive SLR flexion (minimize pelvic torque); sit on level supportive surface; no additional lifting until off wheelchair/assistive devices; avoid max hip flexion/adduction/IR; exercises should not increase pain
Timeline & Phases
Phase 1 (0-2 wks): TTWB, TA isometrics/marching, glute sets, supine isometric clams; Phase 2 (4-8 wks): TTWB->WBAT, closed-chain exercises (sliders/squats/mini lunge), scar/piriformis release, proprioception; Phase 3 (8-12 wks): normalize gait/stairs, 2-leg CKC strengthening, walking/swimming/recumbent bike
