- Weight-Bearing
- NWB x 2-4 wks (younger/athletic, lesion <1 cm, central/posterior talus); NWB up to 6 wks (anterior lesion, smokers, high BMI, older age, lesion >1 cm); progress to WBAT in normal shoe with lace-up ankle brace at wks 6-12
- Key Restrictions
- Ankle ROM limited to gentle AROM in sagittal plane x 6 wks; slower progressions for lesions >1 cm and microfracture; no running until 12 wks; RTS 4-6 months; refer to Ankle Return to Sport guideline
- Timeline & Phases
- 4 phases: Acute (0-6 wks, NWB, AROM sagittal plane only), Post-Acute (6-12 wks, progressive WB, resistance training), Advanced Strengthening (12-24 wks, return to running, sport-specific drills). RTS 4-6 months
