Tendon Transfer — Lower Trapezius Tendon Transfer

Gunslinger brace worn except during exercises x 8 wks; no active or active-assistive ROM until cleared by surgeon; overhead reach only (gravity-assisted) 2-6 wks; scapular mobility begins at 6 wks; 5-lb lifting restriction Phase II; no resisted training Phase II; avoid AROM in positions of subacromial impingement
Timeline & Phases
Phase I (0-8 wks): gunslinger brace, PROM ER to tolerance, overhead reach 2-6 wks, PROM scapular plane elevation to 120 at 6 wks, D/C brace at 6 wks; Phase II (6-12 wks): AAROM/AROM all planes (emphasize all except IR behind back until 3 months), scapular stabilizers, no resisted training; Phase III (>12 wks): progressive RC/shoulder strengthening (isometrics at 3 months, isotonic at 4 months), simulate work/recreational activities
