Phase I (wks 1-2): WBAT locked brace, passive ROM; Phase II (wks 2-6): brace locked, avoid active extension; Phase III (wks 6-12): wean brace, extension 0-30° only; Phase IV (wks 12-16): walk/jog; return to sport wks 20-24
Notes
Medial patellar glides emphasized throughout rehab. Extension arc restrictions protect tibial tubercle fixation.
