Sling 4-6 weeks; no active elbow flexion for 3 weeks (coracobrachialis/biceps detachment); Phase I ROM: 0-3 wks flexion to 90°, ER to 20°; 4-6 wks flexion to 135°, IR to 50°, ER to 30°; if remplissage: limit eccentric infraspinatus strengthening
Timeline & Phases
Phase I (0-6 wks): sling 4-6 wks, no active elbow flexion 0-3 wks, restricted ROM by sub-phase; Phase II (6-12 wks): full AROM, progressive ER, RC strengthening 0-45°; Phase III (12 wks+): <10% LSI strength, ER ≥90° at 90° ABD; Phase IV (16-18 wks): plyometrics, sport biomechanics; Phase V (24+ wks): RTS (overhead: 100% Biodex/ASH/grip; contact: +CKCUET)
Notes
No active elbow flexion for 3 weeks post-op due to coracobrachialis/short head biceps detachment. More restricted initial ROM than Bankart (flexion to 90° only weeks 0-3). Coracoid bone-block procedure for >20% glenoid bone loss. Same RTS criteria as 2025 Bankart version. Copyright 2025.
