Knee immobilizer 4 wks; TDWB 4-6 wks; CKC 20-70° or full extension only; avoid submaximal CKC 8 wks; active hamstring at 6 wks, resistive at 8 wks; no lateral exercises 10 wks; no pivoting/ballistic ≥4 months; no resisted leg extension at any point
