6 phases: Patient Education (pre-op), Maximum Protection (0-6 wks), Protected Motion (6-12 wks), Motion and Muscle Activation (12-18 wks), Advanced Strengthening (18-24+ wks), Advanced Movement and Impact (6+ months). Level 1 ~4-5 months, Level 2 ~9-10 months
