- Weight-Bearing
- 50% WB with crutches 0-2 weeks; advance to full WB and wean crutches by 4 weeks
- Key Restrictions
- No hamstring stretches until 6 weeks post-op; passive ROM only for hip and knee initially; avoid terminal knee flexion/extension on isotonics until 8 weeks
- Timeline & Phases
- Phase 1 (0-4 wks): 50% PWB progressing to FWB by wk 4, passive ROM of knee/hip, no hamstring stretches; Phase 2 (6-8 wks): gait training, hamstring stretches, active/passive ROM, isotonics limited ROM (full ROM at 8 wks), aquatic therapy, core, CKC; Phase 3 (12 wks+): dry land jogging, isokinetic eval (60/120/180 deg/sec vs contralateral), sport/vocation specific drills; Return to sport with >=80% isokinetic strength vs contralateral (anticipated 6-9 months post-op)
