TTWB w/ axillary crutches 0-2 wks, crutches up to 8 wks; hinged knee brace locked at 45-50 deg: until wk 4 (endoscopic) or wk 6 (open); avoid hip flexion coupled with knee extension (hamstring stretch) throughout; formal PT deferred until wk 4 (endoscopic) or wk 6 (open); no running until Phase III; no sprinting until Phase IV
Timeline & Phases
Phase 1: 0-6 wks (NWB/TTWB, locked brace, home HEP, pool walking at 3-4 wks); Phase 2: 6-12 wks (WBAT, wean brace/crutches, gait training, hamstring strengthening in shortened positions); Phase 3: 12-16 wks (sport-specific, running drills, no sprinting); Phase 4: 16-24 wks (return to sport, sprinting, eccentric loading in lengthened positions)
Notes
Endoscopic protocol is 2 wks faster than open for brace unlock and PT initiation. Return to sport criteria include <10% Biodex side-to-side hamstring deficit at 60 and 240 deg/sec
