- TTWB with axillary crutches x 4-6 wks; 50% WB at wks 4-6; FWB without AD by wk 12
- Key Restrictions
- Axillary crutches TTWB x 4-6 wks; brace at all times x 6 wks; no hamstring stretching or long sitting; avoid combined hip flexion with knee extension throughout; no running or high-impact until wk 16
- Timeline & Phases
- Phase I (pre-op) -> Phase II (0-4 wks: max protection, TTWB) -> Phase III (4-6 wks: 50% WB, wean crutches) -> Phase IV (6-12 wks: FWB, AROM + mid-range strengthening) -> Phase V (12-16 wks: plyometrics, modified Nordics) -> Phase VI (16-28 wks: dry land jogging + return to sport)

- 6-phase 28-week protocol; return to sport at 6 months with 90% LSI isokinetic testing; avoid lengthened hamstring position initially; Nordic exercises begin Phase V; updated 8/2023
