Proximal Humerus Fracture — ORIF

Key Restrictions
Sling with abduction pillow; safe zone: max 130 FF, max 30 ER for 6 wks; if concomitant biceps tenodesis: no elbow flexion resistance for 6 wks; no forced IR until Phase III (10+ wks)
Timeline & Phases
Phase I (0-6 wks, PROM): sling + PROM within safe zone -> Phase II (6-10 wks, AROM): full ROM, AAROM/AROM with good mechanics -> Phase III (10+ wks, strengthening): maximize strength + return to work/sport
Notes
Radiographic evidence of healing required to advance phases; aquatic therapy available from 4-6 wks within safe zone; biceps tenodesis modifies elbow rehab for 6 wks
