Non-Operative — Proximal / Mid-Hamstring Strain (Non-Operative)

Sanford Health

Weight-Bearing
N/A
Key Restrictions
Avoid end-range active/passive hamstring lengthening in Phase I; no end-range stretching if hamstring weakness persists; progressive hip flexion loading; avoid lengthened hamstring position early
Timeline & Phases
Phase I (isometric, hip near neutral) -> Phase II (isotonic in reduced hip flexion) -> Phase III (isotonic in increasing hip flexion, 70-90 for proximal) -> Phase IV (energy storage loading, plyometrics, return to sport)
Notes
Type I mid-substance (sprinting, ~4-5 wk RTS) vs Type II proximal (stretching type, ~8-9 wk RTS); neural flossing if positive slump test; BFR supplementation recommended; revised 4/2024
