- 6 phases: Phase I (pre-op), Phase II (inpatient), Phase III (0-3 wks, 4-6 visits), Phase IV (3-6 wks, 3-6 visits), Phase V (6-12 wks), Phase VI (12+ wks); 7-12 total visits expected

- System-level guideline; criterion-based; ROM goals: 5-90 deg Phase II, 5-100 Phase III, 0-110 Phase IV; NMES/biofeedback recommended; manipulation flagged if ROM not progressing by wk 4-5; includes outcome measures (WOMAC, KOOS JR)
