- Sling 3-4 wks; no AROM wks 0-4; no lifting; no excessive motion behind back; no aggressive ER stretching or vigorous IR strengthening (subscapularis precaution); no lifting >5 lbs Phase III; avoid combined ER + abduction >80 deg Phase IV

- Phase I Day 1-21 (immediate/PROM); Phase II 4-6 wks (early strengthening); Phase III 6-12 wks (moderate strengthening); Phase IV 12+ wks (advanced/return to activity); return to hobbies 4-6 months

- Covers both TSA and hemiarthroplasty; subscapularis healing emphasized throughout; intact rotator cuff required for TSA candidacy; expected elevation up to 140 deg for OA with intact RCT
