- Sling full-time 4 wks then wean by 6 wks; Phase 1 staged ROM: FLEX <90°, ER 15-20°(wks 1-2)/30°(wks 3-4)/45°(wks 5-6) in scaption, ABD 30°/60-80°/<=90°; no push-ups/bench press/pec flys/throwing/overhead Phase 2; avoid wide-grip pull-downs/behind-neck press/wide-grip bench/lateral deltoid raises/overhead triceps press Phase 4; RTS 6-12 months

- Phase 1 (wks 0-6): sling full-time 4 wks then wean by 6 wks, staged ROM limits — PROM/AAROM hammer grip, scapular mobs, pendulums, hand squeezes, elbow/wrist AROM, shoulder isometrics (2-finger), BFR uninvolved/LE; wks 4-6: BFR elbow FLEX/EXT, opposite extremity PREs, LE resistance training; Phase 2 (wks 7-12): d/c sling, ROM goals FLEX 160°/ER 60° at 90° ABD/IR symmetric by 12 wks, no push-ups/bench/pec flys/throwing/overhead — ROM, trunk NWB, scapular strengthening (retractors/upward rotators), shoulder PREs, bike then elliptical/treadmill; Phase 3 (wks 10-12 to 4-6 months): normalize ROM, trunk FWB, push-up/pull-up progression (push-up starts 12 wks), A/P GH mobs as indicated, running progression; Phase 4 (4-6 months): weight training, interval throwing (overhead athletes), avoid contraindicated exercises; RTS 6-12 months
