- Sling 24 hrs/day week 1; remove at home weeks 2-4 (use in crowds); D/C sling weeks 5-6
- Key Restrictions
- Avoid High 5 / Low 5 positions; open repair: no active IR 4 weeks, no IR strengthening 6-8 weeks; if coupled with SLAP repair follow open Bankart guidelines; PROM limits: ER 30° (weeks 2-4), 45° (weeks 5-6); full ROM target weeks 9-12; no driving week 1

- Week 1 (sling, pendulum only); Weeks 2-4 (1-3x/week, 3-12 visits: PROM/AAROM, isometrics, scapular control, rhythmic stabilization); Weeks 5-6 (1-3x/week, 5-18 visits: isotonics, scapular strengthening, gentle dynamic stab, PRN NMES); Weeks 7-8 (1-3x/week, 8-24 visits: 2-handed plyometrics, isokinetics, D2 PNF); Weeks 9-12 (gym program, full ROM, MMT 5/5 goal); Weeks 13-15; Weeks 16-26 (sport-specific, interval throwing program); 11-29 total visits
