Sling with abduction pillow at all times wks 1-6 (remove for exercises/showering); D/C sling at wks 6-8; Phase III AROM only — no PROM, AAROM (no pulleys, supine wand, or wall climb); no flies or pulldowns until 4.5 months; light weight training at 4.5 months post-op; RTS at 6 months
Timeline & Phases
Phase I (wks 1-4): sling with abduction pillow, pendulum exercises under PT guidance, elbow/wrist AROM with shoulder in neutral, hand exercises, stationary bike with arm in sling | Phase II (wks 4-6): sling with abduction pillow, AAROM supine with wand, goal FE to 90° | Phase III (wks 6-8): D/C sling, AROM in pain-free range only (NO PROM, AAROM, pulleys, supine wand, wall climb), goals FE 120°/abduction 90°/ER as tolerated/IR and extension with wand behind back, submaximal isometrics | Phase IV (wks 8-12): AROM and AAROM pain-free (no PROM), light theraband (ER/abduction/extension), biceps/triceps PREs, prone scapular retraction without weights, wall push-ups (no elbow flexion >90°), goal full ROM | Phase V (months 3-6): full ROM, light theraband with increasing resistance, light weight training at 4.5 months (no flies or pulldowns), regular push-ups, RTS at 6 months
