Phase 1 (0-4 wks): sling with abduction pillow, no pendulums, no active ROM, PROM: elevation 90°, ER 45° → 70° at 0°/45°/90° elevation, IR to abdomen, no IR behind back, aquatic therapy 3-4 wks (submerged, slow active motion, no coronal plane abduction) | Phase 2 (4-8 wks): wean sling/abduction pillow, PROM → full ROM, elevation to 140°, ER 40° at 0°/45°/90°, IR thumb to L1, aquatic therapy without ROM limits | Phase 3 (8-12 wks): full ROM target by 8 wks, isometric ER/IR at 0°/45°/90°, rhythmic stabilization, AAROM → AROM elevation, PNF D1/D2, periscapular strengthening, light theraband/dumbbell resistance, aquatic therapy with paddle/webbed gloves, UBE | Phase 4 (12-16 wks): progress strengthening with weight, gym machines, IR/ER at 90° abduction, isokinetics 120-240°/sec | Phase 5 (16-24 wks): sport-specific functional progression, interval sport programs, plyometrics with pitchback, advanced strengthening
