Key Restrictions
Sling 6 weeks; sleep in sling 3 weeks; no active ER/extension/abduction; no resisted biceps (elbow flexion/forearm supination) for 6 weeks; no biceps loading until week 10; no throwing/overhead sports until week 20; RTS (contact sports) no sooner than 5 months
Timeline & Phases
Phase I - Protective (0-6 wks): sling, PROM (flex/scaption 90°, ER 30°, IR 45°), no active ER/ext/abd, isometrics; Phase II - Moderate Protection (7-12 wks): isotonic RC strengthening, proprioceptive progressions, biceps AROM/submaximal isometrics at wk 10; Phase III - Minimum Protection (13-20 wks): full ROM, plyometrics, no throwing until wk 20; Phase IV - Advanced (21-26 wks): interval sports, throwing from mound wks 24-28; Isokinetic testing criteria: ER/IR ratio 70%, bilateral ER 98%, bilateral IR 105%
