Sling 0-4 wks; elbow PROM/AAROM/AROM without resistance 0-4 wks (allows tendon to heal into humerus); no resisted biceps strengthening until 8 wks; return to sports at 6 months if approved

Phase I: 0-4 wks (sling, elbow PROM/AROM no resistance, full shoulder AROM); Phase II: 4-12 wks (AROM elbow with passive stretching, light RC/deltoid isometrics, resisted biceps at 8 wks); Phase III: 3-6 months (progressive strengthening, sport-specific, throwing at 3 months, mound at 4.5 months)
