- 20 lb flat-foot WB 0-2/3 wks (6 wks if microfracture); wean crutches at 3-4 wks (6 wks if microfracture); ROM limits x3 wks: flexion <90 deg, extension <0 deg, ER <30 deg at 90 deg flexion, IR <20 deg at 90 deg flexion, abduction <30 deg; no active open-chain hip flexor activation; no sitting >30-45 min at a time x3 wks; prone lying 2-3 hrs/day
- Timeline & Phases
- Phase I: 0-2 wks (FFWB 20 lbs, protected ROM, stationary bike, isometrics); Phase II: 2-6 wks (WBAT, wean crutches, gait normalization, advance squat/step progression); Phase III: 6-12 wks (full ROM, functional strengthening in all planes); Phase IV: 3-6 months (sport-specific: Alter-G running, agility, cutting, plyometrics; RTS typically 4-6 months, up to 1 year for maximal recovery)

- CPM used if applicable (4 hrs/day; reduce to 3 hrs if stationary bike used); microfracture adds 2-3 wks to WB and crutch timeline
