Phase I (surgery-2 wks): sling, protect incision, grip strengthening, gentle forearm/wrist ROM, walking/Stairmaster with sling on, partial passive ROM; Phase II (2-4 wks): full discontinuation of sling, shoulder strengthening/ROM, gentle elbow strengthening with active motion and submaximal isometrics, PROM advanced to AAROM with eccentric mobile wad stretching, ball squeezes; Phase III (4-6 wks): task-specific functional training, strengthening with weights/bands, end-range/passive overpressure ROM, counterforce bracing, return to throwing/swimming at 12 wks, throw from pitcher's mound at 18 wks
