- Sling worn outside PT at all times; no shoulder IR or horizontal adduction past neutral for ~6 wks; no shoulder flexion beyond 90 deg for at least 6 wks; no shoulder IR with arm abduction for at least 8 wks; no pushing or CKC through shoulder for at least 12 wks; sling wean no sooner than 6 wks; CKC (incline/quadruped plank) no sooner than 12 wks; jogging begins 12 wks

- 5 phases: Phase I (0-3 wks), Phase II (3-8 wks), Phase III (8-12 wks), Phase IV (12-18 wks), Phase V (18+ wks); 5-7 months formal rehab (6-9 months for overhead/sport)
