Sling x 6 weeks in neutral rotation (not IR); no IR x 6 weeks; maintain shoulder out of IR throughout early phases
Timeline & Phases
Phase I (1-2 wks): sling in neutral rotation, pain control; Phase II (2-4 wks): AAROM flex/abd to 90°, ER to 45-60°, NO IR x 6 weeks; Phase III (4-6 wks): AAROM flex to 140°, abd to 90°, ER to 60°+, NO IR; Phase IV (6-9 wks): D/C sling, full AROM, rotator cuff strengthening

Posterior instability repair; shoulder maintained in neutral rotation (not IR) in sling; no IR for 6 weeks to protect posterior repair
