PROM only below 90° elevation x 4 wks; AAROM/isometrics at 4 wks; AROM at 6 wks; strengthening at 10 wks; no biceps strengthening (elbow flexion/supination) x 12 wks; Type IV: no isolated biceps contraction or heavy carrying x 12 wks; avoid bench to neutral/military press behind head/lat pulldowns behind head to protect repair
Timeline & Phases
6 phases: Maximum Protection (0-2 wks, sling, PROM <90°), Protected Motion (2-6 wks, sling, progress PROM to 145°/ER to 45°), Motion and Muscle Activation (6-12 wks, AROM at 6 wks, strengthening at 10 wks, biceps at 10-12 wks), Advanced Strengthening (13-24 wks, plyometrics, throwing), Return to Activity (months 6-9, interval throwing). Expected D/C 16-24 wks; RTS 6-9 months
