- Sling 4 wks; no active ER/extension/elevation Phase I; no isolated biceps activation; no resisted supination or elbow flexion 0-6 wks; no biceps strengthening until 6 wks; no lat pulls behind head; never drop elbows below plane of body

- Phase I 0-4 wks (protective/sling); Phase II 5-7 wks (intermediate ROM/early strength); Phase III 8-12 wks (progressive strengthening); Phase IV 12-16 wks (plyometrics); Phase V 16-20 wks (return to sport); interval sports per MD

- 5-phase protocol; therapist must clarify surgical approach (debridement vs. repair) and LHBT status (intact, tenotomy, or tenodesis) as this guides rehab; biceps re-education emphasized 6+ wks
