Hip Arthroscopy / Labral Repair — Revision Hip Arthroscopy
University Hospitals · Mario Gastaldo, DPT, OCS; Kayley Mitchell, DPT, OCS; Kayla DeGeorge, SPT…
- Weight-Bearing
- 50% FFWB first 2-4 weeks, avoid toe-touch WB (caution until nerve block wears off)
- Key Restrictions
- Crutches immediately post-op, weaned based on hip extension ROM/heel-to-toe pattern, goal D/C 21-28 days; brace 0-90 deg ROM, always worn except CPM/shower/HEP, goal D/C 14 days; increased caution with end-range mobility/aggressive ROM vs. labral repair; expect increased pain/slower progress - emphasize education and activity modification
- Timeline & Phases
- Phase I (0-2wks): reduce inflammation/pain, scar management, restore posterior chain/quad neuromuscular control, normalize gait w/ assistive device; Phase II (2-6wks): achieve full symmetrical AROM/PROM, normalize gait and D/C assistive device, progress hip/core strengthening; Phase III (6-12wks): correct compensatory patterns, restore single-limb/multi-directional strength (>80% LSI), begin plyometric progression; Phase IV (12+wks): improve power/endurance, multi-planar stability, return-to-running progression (Appendix A) and return-to-sport testing (Appendix B)
- Notes
- One PDF covers multiple hip arthroscopy procedure variants with distinct precautions (see Surgery Type) sharing a common phase-based rehab framework; PT begins post-op day 1; CPM 4-6 hrs/day x21 days (5-10 deg/day increase); brace worn 24/7 except CPM/shower/HEP; BFR and NMES used throughout; includes phased return-to-running progression (Appendix A) and Hip Labral Repair RTS testing battery (Appendix B: HOS ADL/Sport, hip sport cord test, single-leg bridge test, hop testing, HHD strength, ADD:ABD ratio); capsular plication (Dr. Karns) requires modified precautions: TTWB 5-7 days then heel-toe partial WB x4wks, hip kept slightly flexed x4wks, avoid extension stretching until 12wks
