ER in scapular plane: ≤15 deg wks 1-2, ≤30 deg wks 3-4, ≤50 deg wks 5-6, 90 deg wks 7-9; excessive ER above guideline is a precaution; excessive biceps loading is a precaution; no cross-body motion; no shoulder extension with elbow extension; review operative report before initiating rehab — includes reattachment, debridement, or combination; risk factors: overhead activity, FOOSH, contact sports, poor shoulder strength and scapular stability, faulty mechanics
Timeline & Phases
4 phases: Phase I Weeks 1-2 (PROM: elevation 75 deg scapular plane, ER ≤15 deg; scapular stabilization; submaximal isometrics — no elbow flexion), Weeks 3-4 (PROM: elevation 90 deg, ER ≤30 deg; AAROM begins), Weeks 5-6 (elevation 145 deg, ER ≤50 deg; AROM full can; IR/ER tubing), Phase II Weeks 7-9 (full flex/ABD, ER up to 90 deg), progressive strengthening and return to sport phases
